- Rhabdomyosarcoma Requiring Ovarian Transposition Release for Recurrent Severe Ovulation Pain Following Laparoscopic Ovarian Transposition: A Case Report.2025 · Case: Ovarian transposition (OT) is performed to preserve ovarian function in patients undergoing pelvic or abdominal radiotherapy. Although complications, such as ovarian torsion and cyst formation, have been reported, ovulation-related peritoneal irritation requiring surgical intervention after pediatric OT has not been documented. In this case, a 12-year-old girl who underwent bilateral OT at the age of 6 years during treatment for recurrent rhabdomyosarcoma presented with severe pain in the right lower quadrant. Owing to prior pelvic radiotherapy, the assessment of menstrual history was unreliable. Considering the young age of the patient and the absence of a definitive diagnosis, hormonal therapy, such as low-dose estrogen-progestin therapy, was withheld. Conservative management with analgesics was initiated; however, the pain persisted and progressively worsened. Outcome: Emergent laparoscopic OT release was performed for diagnostic and therapeutic purposes, owing to the severity of pain. Intraoperative findings revealed corpus luteum in the retracted right ovary. Postoperatively, the patient's symptoms resolved immediately without recurrence. Conclusion: Ovulation-induced peritoneal irritation should be recognized as a potential postoperative complication following childhood OT. In adolescent patients with a history of pediatric OT and pelvic radiotherapy, ovulation-related complications should be carefully considered during the differential diagnosis of acute abdominal pain.
- How Effective Is Cervical Ripening Using the Controlled-Release Dinoprostone Vaginal Delivery System for Primiparas with Unruptured Membranes?2024 · Background: Labor induction has a low success rate, especially in primiparas with unruptured membranes. Previous studies focused on pregnant women with unruptured membranes, but none specifically targeted primiparas. Aims: To compare the effectiveness of a controlled-release dinoprostone vaginal delivery system for cervical dilatation (PROPESS) with that of mechanical dilation for labor induction in primiparous women with unruptured membranes. Materials and Methods: We retrospectively analyzed the data of 90 primiparas with unruptured membranes (41 and 49 in the PROPESS and mechanical dilation groups, respectively). The primary outcome was the cesarean section (CS) rate. The secondary outcomes were the prevalence of vaginal delivery within 12 or 24 h after the initial insertion, oxytocin usage rate, chorioamnionitis, additional use of mechanical dilation in the PROPESS group, and neonatal outcomes. Results: The CS rate was significantly lower in the PROPESS group than in the mechanical dilation group (p = 0.02). A total of 13 patients (31.7%) delivered within 24 h with PROPESS alone, indicating a significantly higher rate of delivery within 24 h in the PROPESS group (p = 0.02). Fewer patients required additional oxytocin in the PROPESS group than in the mechanical dilation group (p = 0.001). However, 14 (34%) patients in the PROPESS group required additional mechanical cervical dilation, resulting in a longer time to delivery than mechanical dilation. Conclusions: PROPESS significantly reduced CS rates and increased delivery rates 24 h after the initial insertion in primiparas with unruptured membranes compared to mechanical dilatation. However, failure to respond to PROPESS resulted in an overall longer delivery time than that of the conventional mechanical dilation group; therefore, identifying predictors of response to PROPESS is necessary.

- ヒト卵子形成および胚発生における卵胞液中・血中脂肪酸の影響2016 · 本研究の目的は、卵胞液中のPUFA(多価不飽和脂肪酸)の濃度が、卵子の質や受精、胚の質との関連を明らかにすることである。2016年8月から2017年12月の間に、体外受精と胚移植の対象となる28人の女性を対象に前向き研究を実施。28個の血清サンプルと140個の卵胞液サンプルを収集し、PUFA分画をガスクロマトグラフィーを使用して分析した。 各患者の6つの連続した卵胞から2mlずつ個別に吸引し、卵胞から採取した卵子のその後の培養状況と一致させた。ある種のPUFAは受精の有無によって有意に高値、または低値であり、卵胞液中のPUFAの濃度は、受精において重要な役割を果たす可能性がある。
